So I did a bit of research and only one insurance company in the entire United States covers the procedure (That I could find). But, this insurance (Kaiser Permanente) isn't offered in my state. It is however offered in Georgia, which is relatively close. So my question is, could I switch to Kaiser Permanente if I don't live in Georgia, California, or one of their other locations? It's orthognatic surgery, in case it's relevant.

Dental Insurance Company Denying Authorization for Covered Plan Services.?

Anthem Blue Cross Dental Net I was referred to an oral surgeon by my primary care dentist since I have to have 6 teeth and 2 residual root tips extracted in advance of getting a full upper denture. All the extractions including the root tips have to be done during one visit, which necessitates the use of IV sedation. My general dentist cannot perform any of this work. Anthem Blue Cross Dental Net denied the original pre-authorization request, and subsequent appeal stating that my general dentist can perform the services being referred. Which is absolutely not the case. I even spoke with a customer service rep who indicated that the extraction of residual root tips is definitely a referable service and it clearly states that in their Dental Net Provider Contract. She has no explanation for why the appeal was denied. My primary care dentist has even provided Anthem Blue Cross Dental Net with his specific reasons for referring this work to an oral surgeon and I still can't get anyone at Anthem Blue Cross to re-evaluate the original denial and the appeal and authorize the necessary work. I have filed a complaint with the California Department of Insurance, although I don't hold out much hope for any results from that effort. HELP... what are my options? Who can I contact? Thanks

I was issued a speeding ticket today in Illinois (cook county) and I have a question regarding one of my options: If you are 18 years or older, to avoid a conviction on your driving record, plead guilty, register for traffic safety school, pay $115. So let's assume that I pay the fine and complete traffic safety school. My question is regarding my insurance company...do they see that I was issued a speeding ticket but completed traffic safety school or is there no trace of it at all? I've read many things about this; some say that the ticket remains on record but simply does not count against you. Others say that the ticket will be on record and the insurance company can still choose to raise your rates but there is no penalty with the DMV for speeding. I'm hoping that the ticket is completely thrown out as if it never happened but I'm not sure. I have state farm insurance by the way.

I've carried my auto and homeowners insurance with State Farm for about 15 years. I've had multiple auto claims but only one claim on my homeowners poicy. I had my roof replaced about four years ago when most houses in the area were damaged by hail. I've been very happy dealing with state farm but I'm considering a change to travelers. Travelers sent me a quote that if accurate reduces my homeowner insurance to about $900 per year as compared to the slightly lower benefits from State Farm which is costing $1200 every six months. At the end of the summer I am allowed to buy my own car. At that time I will have been driving for about a year and a half under my parent's insurance. I have 2 options as far a buying a car: putting about a $5000 down payment on a car and my parents paying off the rest before I graduate college, or buying a car for $5000. The cars I would buy for $5000 would either be a mitsubishi 3000gt or a nissan 300zx. I like both of these cars, but they're not very professional and since I hopefully will have whatever car I buy through college, I want something that doesn't scream hey i'm fresh out of high school and stuck with a sports car! As far as putting a $5000 down payment at a local dealership, my limit is about $16,000 at the most. For 16,000 I was looking at a used (obviously) BMW M3, Mazda Rx8, and Audi A4, or (if I get extremely lucky and stumble upon a) Infiniti G35. These cars look a little more mature than the ones I would buy for $5000.

I applied and a day later I was approved for $150. They said they would deposit the cash into my account, nothing has been deposited and this was over 48 hours ago. I have a car that is worth no more than $1500. I bought it off someone in my family and I should transfer ownership (as it is from a different state). Lets say on an average a person pays 150/month for coverage. Over time this money adds up. After one were to be in a collision, they are presumably covered , yet a deductible is payed, then they increase the payments. I fail to see the purpose when they are just pretty much using your monthly payments to pay for these fines. Lets say you had a glass jar, and you put 200 a month in there, is the concept not the same? Even if you were to never get into an accident in your lifetime, do you see a dime of your money? I just really want to know if there is a true reason, or purpose for insurance, because I fail to see past it as a scam.

There was alot of traffic being it was around lunch time and i was approaching an intersection when this dodge durango came out of nowhere and tried to cross in front of me. My dad and his girlfriend have insurance coverage but I'm not listed under their insurance policy as an occasional driver simply because my driving of their car isn't routine and it's not worth the extra expense. So technically, this would be considered as me borrowing their car. Because I don't have my own personal insurance coverage (other than the occasional insurance with my mom), does that make it illegal to drive my dads car or is it perfectly legal for me to borrow his car at random times? I have seen some health care plans for $100 a month and up, but I can't afford that. So, in the process of getting a new vehicle I took two VIN numbers to a man at the insurance agency I've used for the past three years. The two numbers were for an '08 and an '09 car (the '08 first just to get a reference). He tells me the approximate increase in my monthly bill will be around $47.50 (for the '09). HI All, my relative incidentally hint the few new motors in in a motor shop which selling new motor. The standing motors fall down each and another. Lowering my car insurance premium by putting my car under my roommate's policy?

I'm 20 years old and I own a car. I pay a lot for my own insurance. But I live with a 25 year-old roommate. Is there anyway I can lower my insurance premium by putting my car under his insurance policy because we live together in the same household. What do I need to do? We don't have the same insurance company and does that mean I have to terminate my current insurance and switch to his company? Or a lot more need to be done? I have no clue. This means big savings for me so please help. Thank you!

So I was recently given a citation during a trip to Northern California. Here's the condensed version; I went out with a group of friends to have dinner and drinks. It was about 10 people altogether, three car loads. I was originally not one of the drivers (I flew in from Southern California). Several of my friends/acquiantances drank heavily during dinner. I was asked as a favor to drive one of the car and the car owner's home. The car I drove belonged to my good friend's neighbor. I received a speeding ticket during the drive to the neighbor's home (73 in a 65) PLUS no insurance!!! I informed the officer that it was not my car and that it belonged to the passed out lady in the back but he didn't care and decided to write me the ticket anyway. Now I've been told that I will have to pay for it ($988) plus my speeding ticket. I spoke with my good friend about it but the neighbor now refuses to go to court with me or write a letter at the least explaining the scenario.

Today my girlfriend asked me to drive her home from college as she was sick. I used her moms car. When coming home on the arterial we were at a red light and I was getting ready to get in the right lane when the light turned green the SUV in front of me began to go then stopped suddenly, (still don't know why) As I was still crossing into the other lane the corner of my front bumper bumped her fender corner. I noticed she stopped and was on her phone so I backed up and parked behind her assuming she just wanted to make sure it was ok. Her tail lights were fine, she drove Subaru SUV so she had a pretty solid buffer there really wasn't even a scuff but a small red reflector on the bottom of the bumper that received a small crack (got a pic close up but can't see it more than a couple inches away. My headlight just has small scuff we werent worried, in NYS you don't generally call unless the damage is over 1000 but she insisted on calling the police. I kept asking if she'd like to just exchange insurance info but she said no (stubborn middle aged probably for insurance) The cops kept telling her we could just exchange info but she wanted report. Anyways, we sat there for about an hour and eventually just got the accident report and I got a following too closely ticket. Do you know how much something like this would cost in NYS roughly and if it would add points on my license. (Cant have pts for work ) Also how will insurance be affected?

For example, lets say I have car insurance and pay it on a timely matter and then all of a sudden I move to city where there is a lot of public transportation and my car breaks down. I am not sure who is right on this one.... We bought a car in 12/11. We purchased gap insurance and an extended warranty at that time through the dealer. We decided to refinance the car through the credit union were we are members. The gap insurance was 1/2 the price we purchased at the dealer. The dealer and bank said that we should receive a refund. hi all i was hoping you could get some facts/and or legal advice. Ok so i have been renting this car for about 6 month now because i cant afford a down payment for my own. I had my own car insurance which covered the rental. However(bad luck struck) i got into a car accident 2 days after my car insurance lapsed i have since renewed my policy right away with another insurance company that gave me better rates. The fault of the accident has not yet been determined. I believe it was the other persons fault. however i am worried what will happen if it is deemed to be my fault. i was not covered at the time of the accident, i did not obviously have coverage through the rental company. i want to make something clear that i am a person of good faith and if it were to be deemed as my fault i have no problem paying the other persons deductable. but i am worried about someone going after me for thousands and thousands or jail time or loosing my licence. so my questions are do rental car companies carry default uninsured motorist coverage(even if i opp out of purchasing theres?) if it is my fault do i get to pay the other persons deductable(and damage to rental) and walk away? will her insurance sue me for full cost of repairs? will her insurance go after rental company go after rental place for money? will her insurance go after my new insurance even though i was not covered by them at the time of the accident? fyi: no one was hurt, she has a minor medium size dent in door panel, i have small tear in my bumper

They were touting their high level of service and low prices, and urging consumers to investigate and switch to their company. How come this doesn't appear to happen with health insurance? I recently switched car insurance companies (from country insurance to allstate) and when my husband called to cancel our old companies policy they refused until we fax them a copy of our new policy with allstate. I dont understand why thy need this information. But they said they cant do anything about it, If I want I can raise this with the customer service chief and chief executive. a friend of mine is an american citizen living in Juarez, Mexico because of his wife is a Mexican citizen that can't cross until her papers are finalized.. So he works here and goes back to Juarez for the night and comes back in the morning.. the other day on his way home.. he stopped at the store and when he walked out.. someone stole his truck.. he called the police and reported a stolen truck but with no info given about himself or the truck, just merely a black suburban... now he's back in the states.. and wants to report it stolen here from a friends house or something... not at a store because walmart has video cameras in their parking lots.. so how can he proceed in doing this.. he doesn't want to cause insurance fraud but he has nothing left besides his truck.. if he tells the insurance company it was stolen across the border, there is no policy on where it was stolen, so Juarez, Mexico is out of their jurisdiction.. how can he do this without getting in trouble.. he's worried about the border itself.. because they snap a photo of you coming into the united states.. but do they snap a photo of you when they get out? any info on what he can do to help his situation would help so much.. thank you

Would I still need to keep the insurance up on my truck if my license was suspended for DUI?

I was wondering if I still need to keep my insurance during the suspension period or do I cancel it and then wait till the end of my suspension period. I got my DUI on May 6th. I guess for now I'm sticking to riding a bicycle anytime I want to go somewhere which it takes longer but at least it gets the most gas mileage of any vehicle which is infinity miles per gallon.

The Car insurance company gave me a low estimate on repairs?

Long story short. Not my fault. I can not take this up with my insurance company because I don't have one. We got his info. My car has 140K miles on it. It's worth about 2K. I was stopped at a red light behind two cars. Both cars started to go and so did I the next thing I knew I had hit the car in front of me. The car in front of me hit the car in front of him. We all pulled over my car isnt to bad but I drive a suburban, the car in front of me was a smaller car so his looked a little worse. Nobody was hurt, and all cars were drivable, so we decided to not call the police we just all exchanged information. After we had finished and they left I immediately called my insurance lady to let her know what happened. I am not super clear on this next part, we have been going back and forth with our insurance since that call and will continue to fight it. When I called the insurance lady she said I wasnt covered. She said that they had received the last payment much later than it was due (not sure how because it was mailed the same time as always) but because of that our insurance had been cancelled. A couple of weeks ago they had chased our check, she said we were reinstated as of yesterday (the day of the accident) but because it was the same day they werent liable to pay the damages. We are going to keep fighting them because I dont think its fair. Even if they decide that they wont pay we can afford to just pay for the damages to the rear end of the car in front of us, because know matter what happened I am at fault for following to close. We cant afford to pay out of our pocket also for his front end and the rear end of the first car. Shouldnt those damages be covered by the car I hit because he was obviously following to close to the car in front of him? Im a little freaked out with all this, this is the first accident I have ever been in and we have always had insurance, so we thought. The insurance company did say they mailed us a cancellation notice but if they did we have never received one.

My wife and I are in our 40's and 50's. We have created A/B trusts and transferred our assets to them, and expect to have estates that will benefit from the estate tax saving these trusts afford. We recently both purchased $1M 10-year renewable term life insurance policies that we expect to keep for 10 years. We chose not to use life insurance trusts because the purpose of the policies is to replace the lost income of the deceased spouse if one of us dies relatively soon, not to increase our final estate, and the trusts are somewhat costly. We each specified our own trusts as the beneficiaries, because we want the disposition of assets specified there to be used for this money. The trusts provide for the income, and principle if needed, to be paid to the spouse, and for the remainder to be paid to our child at the remaining spouses death. The question of ownership of the policies is less clear. My lawyer says that it doesn't make much difference who owns them, because the value (estate) taxed is only one year's premium, not the $1M. My accountant says that the $1M is taxed, and that each policy should be owned by the other spouse to avoid tax on that amount. A friend said that there's no way to avoid or significantly decrease the tax, so just leave the ownership as is (each spouse owns their own policy). A further fly in the ointment is that the insurance company (Massachusetts SBLI) makes some forms of trust ownership more difficult and costly. I'm confused. What ownership is the best? How to insure a used car off of the lot with no previous insurance?

I have never had insurance in my name before (my parents had me under their insurance within the past 6 months, but I am now not insured, nor driving a vehicle). I am interested in buying a car from a buy here, pay here lot and I don't quite understand how it works as far as insuring the vehicle off of the lot goes. I was told that the dealer would give me a few options of insurance (usually crappy) and I would use that insurance for the drive home, and subsequently get my own insurance. I am in California, was in an accident with no insurance. The police report states it cannot find who was at fault and the lady's insurance company said the same thing when I spoke with them to see if they would pay for my damages. I found the perfect plan for $33 a month, but it has a $10,000 deductible. WTF? Unless I become pregnant and have a kid, or need surgery or whatever, I find it impossible to make a $10,000 bill within a year. That's too much. I've always been healthy and rarely need to visit doctors. Then I was looking at other policies and they were more expensive, but they all had deductibles. One was like $160 a month with a deductible of $5,000.
